Finding the Target Dose: Start Low, Titrate Gradually

There is no universal dose for cannabinoids. The target is reached by starting low and increasing gradually until the desired effect, with the prescribing physician defining every step.

Why titration

Treatment with cannabinoids depends on formulation, form of administration and the individual. Response varies with weight, metabolism, endocannabinoid tone and the condition being supported. The practical answer is titration: start with a low dose and increase in small steps until the target defined by the physician, or until the desired effect is reached.

A common titration pattern

  • Days 1 and 2: a low starting dose in the late afternoon or evening.
  • Days 3 and 4: if well tolerated, add a morning dose or increase the evening dose slightly.
  • Days 5 and 6: continue increasing gradually, observing response.
  • Follow-up: if side effects appear, return to the previous dose and reassess with the physician.

The dose-response curve

Effect does not grow indefinitely with dose. Many patients find their best response in a middle range; going above it can reduce the benefit or add side effects. This is why monitoring matters more than reaching a high number.

Sublingual oils

Place the drops under the tongue, hold for 60 to 90 seconds and swallow. Onset is usually 15 to 45 minutes, with effects lasting 4 to 6 hours. US oils are calibrated at 40 drops per mL; Swiss oils at 20 drops per mL. Always use the dropper calibration printed on the product.

Oral formats

Gummies and softgels are absorbed through the digestive tract: slower onset, commonly 45 to 120 minutes, with a longer and steadier effect. Wait for full onset before considering another dose.

Topicals

Applied to the skin, topicals act locally without meaningful systemic absorption. They do not change the oral dose and can be reapplied as needed.
